㋡ Hospitalization Procedure
✎ Reservation
1. Once you are advised hospitalization by your doctor with the admission certificate,please contact us for your reservation.
2. The staffs of PTCH will help with your admission process and email you when your admission date is fixed.
3. Here are the procedures on the first day of hospitalization. (1) Please go to admission reception desk at the notified time and date. (2) Please submit your patient ID card and requested documents to the nurse station.
4. A name band is placed on the patient’s wrist to correctly identify the patient and be able to quickly confirm the patient’s name. Then the nurse will run some medical tests for the patient to check the blood pressure, take X-rays, electrocardiogram and a blood test.
5. After completing your admission process, the nurse will bring the patient to the appointed ward.

✎ Hospitalization Reminder (Items to Bring for Your Hospital Stay)
Personal seal, basic items such as toiletries, cups, chopsticks, towels, bath goods, false teeth, tissues, slippers. etc.
*Daily necessities are sold at a store in the ground floor.
*Beddings (blankets, sheets and pillows) are provided during your hospital stay.
*To avoid theft, do not bring a large amount of cash and valuables.
(ATM of Taiwan Cooperative Bank and Mega International Commercial Bank are located on the first floor near the Wellcare Store and the Emergency entrance.)

Art.1: Our hospital treats all patients equally and does not treat them differently depending on the race or age.
Art.2: Our hospital will make sure patients have professional medical service in a safe medical environment.
Art.3: The patients will understand the whole accurate medical information and health education.
Art.4: You can get the relevant information and medical services such as the hospital's consistent education guidance and home care after discharge.
Art.5: Uphold the concept of “patient as the center of medical treatment,” during the hospitalization, the doctors of this hospital should explain the condition, test; check related information, treatment policy. Also, the doctors should inform you in advance about what might happen after the treatment. If you have any questions, you are encouraged to consult doctors or medical staff.
Art.6: If you need surgery, our hospital will first ask you or your spouse, relatives or related persons to sign an operation and anesthesia consent form. Before signing, the doctor will first explain the cause of the operation, the success rate of the operation or Possible complications and dangers will only be treated for your surgery and anesthesia, with the consent of you or your spouse, relatives or related persons. However, if the situation is urgent, to save the patient's life, according to the medical law, the patient must be operated first or necessary.
Art.7: The Hospice Palliative Care Act is specifically stipulated to respect terminal illness patients’ will on the medical treatment, and protect their right. Our hospital provides “Advance choosing Hospice intent form,” “Medical decision maker authorization form,” “DNR consent form,” “Withdraw Hospice intent form declaration form.” If you need any of the forms above, please contact the nursing station. To enable physicians, nurses, and other medical personnel to provide palliative and supportive medical care measures to alleviate or relieve their pain, in the absence of enthusiasm for treatment or first aid for patients who are incurable.
Art.8: The hospital appropriately keeps all medical records and health records of patients' medical treatment in this hospital and respect all patients' privacy.
Art.9: Our hospital strictly keeps patients’ health and medical information in confidential by law. If you are unwilling to let visitors know about your hospital stay, please let us know.
Art.10: The medical staff of the hospital are all wearing identification certificates and practicing licenses. If anyone does not wear a badge and a license, patients can ask them to wear or explain in order to protect patients’ rights.
Art.11: To make the limit lives become limitless, our hospital follows the government’s health policy, and provide the organ donation consent. This consent can help patients’ family members fully understand patients’ willing.
Art.12: This hospital is a teaching hospital. In order to promote medical education and train excellent medical personnel, you are kindly requested to cooperate with relevant teaching activities. However, you have the right to refuse any non-related examination to treatment. Your refusal will not affect the attitude of the medical staff of the hospital to your service and the quality of medical care provided.

Art.1: When you seek medical attention, please follow the instructions of the medical staff to register first.,and please show your ID card(Health ID or ID card);Please participate in medical decision-making with your family during medical treatment.;No medications prescribed by non-physicians may be given to the patient during hospitalization without the permission of the physician.
Art.2: The hospital is completely non-smoking, chewing betel nut. Prohibiting the use of mobile phones in certain areas. And do not taking pets to hospital to prevent infectious diseases and disturb the peace of the ward..
Art.3: Please cooperate with the maintenance of hospital ward safety and tranquility, avoid loud noise, prohibit gambling, drinking, etc., so as not to affect other patients' rest. Please cooperate with the visitor to visit the patient during the meeting time. It is strictly forbidden to break into the computer room, warehouse, roof, other wards or other office areas.
Art.4: Therapeutic diet should be in accordance with the regulations of the attending physician. If you have a non-therapeutic diet, please ask the nursing station. (Please refer to the relevant regulations of this hospital for meal expenses and ordering time.)
Art.5: During the hospitalization, if you need to leave the ward temporary, please inform the nursing station to prevent infecting the treatment. If you want to leave the hospital temporary, please get the doctors permission, and take the leave form to the nursing station to apply. According to the NHI Regulation, Act16,hospitalized patient cannot stay overnight outside the hospital. The time for leave should not exceed 4 hours, and only one time for each hospitalization. If you leave the hospital without a leave of absence, the hospital will be deemed to be discharged automatically.
Art.6: To protect ward safety, people shouldn’t cook in ward, bathroom, or other area. If there’s any requirement for food requirement, please purchase food at the shop on the 1st floor. Please don’t use any medical appliance which is not provide from the hospital to maintain the safety of public electric use.
Art.7: To maintain the security of your property, please pay attention to any items on your side in order to avoid theft and prohibited valuables to the hospital. If you found anyone acting suspiciously, please notify the nursing station at any time.
Art.8: In order to avoid infection, children and the elderly should try to avoid entering the ward; if necessary, please cooperate with the relevant regulations of the visitor limit of the hospital.
Art.9: The hospital absolutely prohibits anyone from carrying dangerous goods and contraband into the hospital, and all offenders are forced to discharge or report to the police.

1.There are water dispenser, fridge, and kitchen waste recycling bucket in the kitchen of each nursing station.
2.On the 6th floor of Han-Wei Building, we provide coin-operated washer and dryer (20NTD per use).
3.Any requirement of wheel chairs or other assistive devices can be registered at the nursing station.
4.Each nursing station has a lounge which provide public TV, table, and chairs. Please keep the voice volume down and the environment clean.
5.There are pharmacy and convenience store on the 1st floor.
6.There are two ATMs in the hospital, one is at the 1st floor, which is next to the lift of Yueh-Han Building, the other one is on the right hand side of ER Exit. There is one ATM on the left hand side of the entrance of Jui Kunag Branch.
7.If you find any facilities in the ward are broken, please inform the staffs, and we will check and repair as soon as possible.
1.To maintain the safety of electricity in the hospital, it’s not allowed to use any appliances like rice cooker, induction cooker…etc.
2.Please prepare pillow and quilt if the patient’s family member wants to stay overnight.
3.Only one family member is allowed to stay with the patient after 10 p.m. If there’s any requirement of a carer, please ask the staff at the nursing station.
4.PTCH declined any presents with appreciation. PTCH also doesn’t allow any drugs or products promotion. If any salesperson approaches, please refuse and inform the nursing station.
5.According to the National Health Insurance Administration, the patient needs to apply to leave the hospital by the doctor’s evaluation and approval. Each admission can only take one leave, the time must not be longer than 4 hours, and the patient is not allowed to stay overnight outside the hospital. If the patient leaves without permission, the hospital would see this patient as discharged automatically.
6.All visitors should follow the visiting hours(7:00-22:00). During the visit, please do not talk loudly, gambling, drinking, or having violence behavior. Using or injecting personal drugs are also not allowed. Please turn off the TV after 10 p.m.
7.ICU visiting hour: Adult - day 11:00 ~11:30 / night 20:00~20:30 Pediatric - day 10:30~11:00 / night 20:00~20:30
8.According to Article 15 of Tobacco Hazards Prevention Act, Smoking is completely prohibited in the medical institution. People who violates the law will be fined from NTD2,000 to NTD10,000.
9.To protect your own belongings, please keep an eye around.
10. If the visitor or patient's family member are coughing or having a fever, please wear the mask. Please don't visit the patient and it's better to see a doctor.
1.During the admission, the hospital provides meals for the patient, please order meals from the nursing station (Ministry of the Interior subsidizes meals of the low-income household.)
2.We only provide congee for the breakfast, if you have any specific request please inform the staff when you order.
3.Daily meals providing time:
Ordering: breakfast(before 6 a.m.)/lunch(before 10am)/dinner(before 4pm)
Serving: breakfast(7am-8am)/lunch( 11am-12pm)/dinner(5pm-6pm)
4.To coordinate of disease treatment, after the doctors make a diet prescription, the dietitian of the hospital will prepare a therapeutic diet suitable for you (such as tube irrigation, high blood pressure diet, diabetes diet), to ensure your food safety, we recommend you order meals from the hospital.
* Health insurance will cover the fee for the patients who are fed by tubes. Other patients would need to pay by their own since meals are counted as self-funded items.

2. Inner transfer fee: During the hospitalization, if a patient wants to transfer to the same standard ward(like transfer to a different bed in the same ward, or transfer from one three-beds ward to another three-bed ward), the hospital will charge NTD200 as a cleaning fee.

Note:
1. Please pay the material fee and medicine fee at the counter before admission (emergency treatment is excluded).
2. During the hospitalization, if the patient receives an “admission payment notice,” please pay the bill at ER payment counter on the 1st floor.
1.Hospitalization:Please apply at the nursing station.
2.Discharge:Please follow the procedure of Outpatient registration.
1.After the patient receives permission of discharge from the doctor, the staff at the nursing station will give the patient discharge notice, prescription, medical bill, and related application(like the certificate of diagnosis). Please bring these documents to the ER payment counter at 1st floor to apply discharge (please bring the Health Insurance card on-hold paper to get your card back).
2.Discharge prescription:After the discharge procedure, please bring the prescription to the pharmacy to get the medicine.
